چکیده

Epilepsy is a neurological condition characterized by repeated seizures, which are clinical manifestations of abnormal electrical discharges in the brain. Epilepsy is the second most common neurological disorder in India. Epilepsy affects an estimated 7 million people in India, and 50 million worldwide. Approximately 40% of them are women. The prevalence of epilepsy is 0.7% in India, which is comparable to the United States and other developed nations. The estimated incidence rate ranges from 40 to 60 per 100,000 population per year. The WHO estimated that approximately 80% people with epilepsy live in developing countries and most of them do not get adequate medical treatment. Among the two distinct epileptic seizures (partial and generalized), the bulk of cases in India belong to generalized seizures. Antiepileptic drugs (AEDs) are the mainstay for the treatment of epilepsy, and generally suppress seizure occurrence. [Table 1] None of these agents, however, meet the characteristics of an ideal AED, that is, protecting against seizures without inducing adverse effects that impair the patient’s quality of life. Despite many advances in epilepsy research, the pharmacotherapy of epilepsy remains largely empirical owing to the lack of understanding of the underlying pathology. Moreover, approximately 30% of people with epilepsy have “intractable seizures” that do not respond to even the best available treatment. Epilepsy is characterized by the unpredictable occurrence Pharmacotherapy of catamenial epilepsy

Catamenial Epilepsy: Discovery of an Extrasynaptic Molecular Mechanism for Targeted Therapy

Catamenial epilepsy is a type of refractory epilepsy characterized by seizure clusters around perimenstrual or periovulatory period. The pathophysiology of catamenial epilepsy still remains unclear, yet there are few animal models to study this gender-specific disorder. The pathophysiology of perimenstrual catamenial epilepsy involves the withdrawal of the progesterone-derived GABAergic neurost...

Diagnosis and management of catamenial seizures: a review

Catamenial epilepsy is defined as a pattern of seizures that changes in severity during particular phases of the menstrual cycle, wherein estrogens are proconvulsant, increasing the neuronal excitability; and progesterone is anticonvulsant, enhancing GABA-mediated inhibition. Thus, changes in serum estradiol/progesterone ratio throughout a normal reproductive cycle bring about an increased or d...

Three patterns of catamenial epilepsy.

PURPOSE On the basis of the neuroactive properties of estradiol and progesterone and the menstrually related cyclic variations of their serum concentrations, we propose the existence of three hormonally based patterns of seizure exacerbation. Because previous reports both support and refute the concept of catamenial epilepsy, we test the hypothesis by charting seizures and menses and measuring ...

Catamenial epilepsy: Definition, prevalence pathophysiology and treatment

Seizures do not occur randomly. They tend to cluster in the majority of men and women with epilepsy. Seizure clusters, in turn, often show a periodicity. When the periodicity of seizure exacerbation aligns itself with that of the menstrual cycle, it is designated as catamenial epilepsy. The neuroactive properties of reproductive steroids and the cyclic variation in their serum concentrations ar...

Catamenial epilepsy: A missed cause of refractory seizure in young women.

Catamenial epilepsy refers to changes in the frequency of seizures over the course of the menstrual cycle. A thorough history and detailed review of the patient's seizure diary are imperative to classify the seizures accurately and select the most appropriate antiepileptic treatment. As catamenial epilepsy rarely responds to antiepileptic medications, the physician should regularly revise the t...
